Prayer Requests: allowing pastors, the prayer team, and/or the congregation to lift up those who need it in prayer.
Chancel Flower Requests: request flowers to be placed in the Sanctuary to honor, memorialize or celebrate loved ones. Flowers are placed in two large vases in the Chancel of our Sanctuary for worship each Sunday morning. Our worship bulletin includes the current week’s flower dedication.
Check Request Form Members can request the issuance of a check for payments such as reimbursements, vendor services, donations, or event-related expenses.
Technology Assistance: Use this form to notify the Communications & Technology Committee of any non-urgent issues related to Wayne Presbyterian Church’s technology platforms, apps, or devices.
Website Issue Reporting allows church staff, members, or visitors to notify the web team of problems or suggest updates to the church website.

Confirmation Inquiry Form: Used for individuals seeking confirmation into the church. It usually includes personal information, baptism history, current church involvement, and reasons for seeking confirmation. Learn more about confirmation.
Funeral Inquiries: Should be directed to the church office. Initial inquiries gather details about the deceased, family contacts, preferred funeral date and location, and any specific religious or cultural wishes for the service. The Room/Meeting Space Reservation Form is designed to facilitate requests for booking meeting rooms for standard meetings and small gatherings. All reservation requests are subject to approval by the facility management team. Please note that this form is not intended for special events, which require a separate process.
